Add a form

Make sure you are in Edit mode (click the dropbox at the top right corner of the page and select edit mode)
On the page that you want to add a form to, click the Manage icon found just above the page title. The Manage lightbox will be displayed.
In the Manage light box, click the Add button besides the Form Builder element.
The Form Builder Controls are now available below the page main text.
